3. Safety & Compliance Training

Have you completed a defensive driving course? List any safety / defensive driving courses completed (with dates if possible): Have you received training in basic first aid / emergency procedures? Are you familiar with vehicle maintenance basics (checks, inspections, etc.)?

4. Company-Specific Requirements

Do you have knowledge of the delivery area (city/region) you wish to work in? Preferred Delivery Areas (cities/regions/neighborhoods): Rate your customer service skills: Are you comfortable using smartphone apps for delivery management, tracking, and communication?

7. Documentation & Insurance Requirements

The following documents are required and must be sent by email to feliciajones@frcourierconceptsllc.com:

Proof of vehicle insurance (if using your own vehicle)
Certificate of Insurance (COI)
Signed contractor agreement (if already provided)
Signed NDA (if applicable)

COI IMPORTANT NOTE: Your Certificate of Insurance (COI) must list the correct company as the Certificate Holder:
• Jones & Sons Construction LLC, or
• F&R Courier Concepts LLC depending on the work being performed.
